Mindstate Psychology (“we”, “us”, “our”) recognises the value offeedback as we continually seek to improve our services. Feedback can be provided through the following channels:
- Formally, via our online Feedback Form
- In writing, submitted directly to us (i.e., by email or post).
- Informally, by making comments to our Staff, Practitioners, or reception team
Complaints are not considered feedback and are not included inthis policy. If you wish to submit a complaint, please use our Complaints Form.
Use of Feedback
Feedback is only used to improve our facilities and services. While valued by us, the provision of feedback is entirely voluntary.
Mindstate Psychology reserves the right to share anonymised and de-identified feedback with our Practitioners, Staff, Contractors, or other third parties as necessary to improve our services and facilities.
Please note that there are no grounds for compensation if Mindstate Psychology incorporates, actions, or otherwise uses any feedback you have provided.
Feedback Review Process
All feedback is reviewed by our executive management team as it is received. In the first instance, feedback is reviewed by our Practice Manager and may be escalated to the Director or other relevant leadership depending onthe nature of the feedback.
Feedback relating to individual staff members or practitioners is handled as follows:
- Positive feedback may be shared with the relevant individual(s)
- Negative or constructive feedback is used to review and improve our training, systems, and processes. It is not attributed to, orshared with, any individual staff member or practitioner — whether named orotherwise
- Feedback about staff or practitioners is not usedas part of any formal performance management process
Please note that we do not respond individually to feedback submissions. If you require a formal response, please submit a complaint using our Complaints Form.
Feedback Privacy
You may submit feedback anonymously where the method of submission permits. However, please be aware that some methods, such as speaking directlyto a staff member or practitioner, may make you identifiable regardless of yourintent to remain anonymous.
All feedback is de-identified and aggregated before storage. Feedback is stored securely by Mindstate Psychology in accordance with both the "Storage of Information" clause and the retention schedule outlined in our Privacy Policy.
As feedback is kept de-identified and aggregated, we are unable to:
- Provide you with access to the feedback you have submitted
- Alter or delete any feedback you havesubmitted previously
- Provide you with a copy of the feedback youhave submitted

Changes to these Terms
Mindstate Psychology reserves the right to update this Policy periodically, and without prior notice to you, to reflect changes either within our practice or in accordance with relevant laws. Any changes to thePolicy will become effective upon publishing the updated version to the Mindstate Website. If we make any material changes to this Policy, we will endeavour to notify you, either directly by email or indirectly, by placing aprominent notice on our Website and/or in our practice, but we will not be liablefor any failure to do so.
